Photo Release:
The Children’s Discovery Museum has my permission to use my or my child’s photograph publicly to promote the Museum. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
Liability Waiver
The Children’s Discovery Museum does not provide medical or accident insurance coverage and is not responsible for personal articles that are lost or stolen. Assumption of Risk, Waiver, and Release By completing this form I am fully aware of the fact these are special dangers and risks inherent in this activity, including, but not limited to, the risk of serious physical injury, death or other harmful consequences that may arise or result directly or indirectly to me and/or students from my participation in this activity. Being fully informed as to these risks and in consideration of my being allowed to participate in Children’s Discovery Museum activities, I hereby assume all risk of injury, damage, liability, and harm to myself and/or child arising from such activities. I also hereby individually and on behalf of my heirs, executors and assigns, release and hold harmless Children’d Discovery Museum, their officials, volunteers, employees, and agents and waive any rights of recovery that I might have to bring to claim or lawsuit against them for any personal injury, death, or other consequences occurring to me arising out of my voluntary participation in this activity.
